Closely works with the outreach teams to develop and maintain a smooth referral system. Beneficiary anthropometric measurement and record keeping at OTP/SFP sites • Conduct quality anthropometric measures at the OTP/SFP sites on the referral beneficiaries following the initial screening exercise while ensuring efficient and effective workflow. • Compile, organize and maintain accurate records of beneficiaries referred to the OTP/SFP sites by documenting all the essential information in a hard-copy according to the standard OTP/SFP recording format and standards of the ACF. • Keep the record of the beneficiaries in an automated (computerized) data processing system. • Problem solve of beneficiaries complaint related to records and admission criteria. • To counsel all the discharged patients for further follow ups through Community Nutrition Volunteers (CNVs). • Work with the outreach teams to develop and maintain a referral system at community level. Engage CNV in referring malnourished children and pregnant and lactating women special attention to the basic health units. • Provide feedback the status of the supplies to the OTP/SFP Supervisor. • Participate in the development of fact sheets and other training materials as required for the MOH health staffs. • Assist the OTP/SFP Supervisor in providing MOH staffs with technical support in convening meetings and answering questions on topics relevant to the project. • Collaborate with the MOH staffs at the health centers for the day-to-day implementation of the OTP/SFP services by developing new measures and/or maintain existing measures as directed. • Update technical specifications for the MOH staff as needed as per the OTP/SFP Team Leader direction. Handling of anthropometric tools and reporting • Check the condition of anthropometric materials received or to be sent, and verify the accuracy of any previously inventory of the tools • Responsible for proper handling of weighing and measuring equipments. Ensure scales are properly calibrated at all times and report defects, damage or loss of equipment to the OTP/SFP Supervisor. • Assist in the preparation of reports about the admission, discharge, defaulter, absent, etc throughout the project cycle. ` Network and coordinate with the relevant sectors • Attend regularly scheduled staff meeting at the health centre as directed and assist by the OTP/SFP Supervisors • Actively participate on workgroups created at the health centre to support the project Centre of Excellence for Rural Development (CERD) NA (Nutrition Assistant) 1st April 2013 till 10th October 2013 Responsibilities • Attaining the appropriate knowledge of anthropometry/health hygiene, SFP, OTP, SC and overall CMAM/IYCF protocols. • Attaining the appropriate knowledge of working area, community, key stakeholders, local nutrition meals, high density recipes and complementary food. • Screening and registering clients in appropriate feeding program as per CMAM protocols. • Providing supplementary and therapeutic foods to the identified clients as per CMAM protocols. • Referring and follow up of Severe Acute malnourished children with complications and no appetite to the identified SC’s. • Promoting Infant Young Child Feeding (IYCF) health practices with more emphasis on exclusive breastfeeding and proper complementary feeding particularly on immunization days. • Educating mother/caretakers on proper use of provided food and medication. • Registering and referring target children and PLW on health for MM supplementation on health facility level. • Distributing MM supplement to target children and PLW on health facility level. • Educating mothers/caretakers on the proper use of MM supplements on health facility level. • Ensuring regular reporting of all activities daily/weekly or as and when required. • Keeping appropriate partnership with DoH staff (LHW’s, LHS and technical LHV’s) Community Uplift Program (CUP) FSO (Female Social Organizer) 22nd June 2007 till 31st December 2008 Responsibilities • Mobilized communities, organizations and partners for management of concern area. • Facilitate formation of appropriate village and valley level conservation forums for co-implementation of the CESSD intervention in the project areas. • Collect and compile baseline information on social, economic and ecological parameters of the project area. • Lead and facilitate the field teams in conducting field surveys, resource need assessment and participatory appraisals in the project valleys. • Write valley conservation Plans and other activity reports on desired format • Keep close coordination and liaison with field teams, communities and partners and assist all in data collection from the field. • Help the technical staff in organizing community meetings, workshops/conservation planning exercises and campaign. • Take lead in resolution and management of relevant community conflicts over common resource utilization in the project area. • Help build linkages of the project community local institutions with partner organization, donor agencies and development players, working both in and out of project area. • Organize training and exposure to improve knowledge and skill of the project communities and partners in sustainable natural resource management. • Document and share lessons learnt achievements and successes from the field.
